1. "Invalid Personal Code Format" Rejection
- This error means that at least one of the personal codes entered is incorrect for the relevant director, PSC, or LLP member.
- If there are multiple officers or PSCs, Companies House does not specify whose code is wrong. You must check each code individually.

- Ask Each Officer or PSC for Proof of Their Code:
- Request a screenshot from their Gov One account showing their personal code.
- Or, ask for a copy of the email from Companies House containing their code.
- Double-check that the code you enter exactly matches the official code.
- If you have verified all codes and still receive the error:
- Send evidence (screenshots or emails) to Inform Direct support so help can be provided in identifying the problem.
2. Other Verification Rejection Errors
- Rejections such as "The verification details for Joe Bloggs are incorrect" indicate that the details on the director/PSC/member record (such as name or date of birth) do not match the ID used for verification.
- Even if the personal code is correct, Companies House may reject the submission if:
- The name, date of birth, or other personal data entered on the record does not match what was used during the ID verification process.
Troubleshooting Steps
- Double-check all officer details:
- Confirm that the name, date of birth, and personal code for each director/PSC/member match the ID they used to verify with.
- Ensure there are no spelling or formatting errors.
- For ACSPs:
- If and ACSP verified the individual, please review the data submitted in the ACSP portal—incorrect or inconsistent details will cause rejection.

- If the personal code, name, and date of birth are all correct, email Inform Direct support with evidence and a description of your troubleshooting steps.
- Include company name, officer details, and proof (screenshots, emails).
